29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a)Smithfield Foods, Inc. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 27, 2024, the employer did not ensure that personal lockout devices were affixed to each energy isolating device prior to conducting line breaking operations for Evaporator 0205. This condition exposed employees to anhydrous ammonia. The following energy isolating devices required a lockout device: (1) Hot gas stop valve VH101-2080; (2) Solenoid valve VO103-0064; (3) MVP stop valve VL101-0205; (4) MVP solenoid stem valve; (5) Pump out valve VP101-2080; (6) Vent valve VV102-0205; (7) Safety relief valve (VF-101-0205)
