29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: Site- Employees working from a pump jack scaffold rated for two workers and one box of siding were not protected from a scaffold collapse hazard where three employees were working from the pump jack scaffold with one box of siding loaded on the scaffold. The scaffold was also erected without all the required bracing and foundation support.
