29 CFR 1910.146(c)(8)(iii): When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not apprise the contractor of any precautions or procedures that the host employer implemented: a) Domtar Paper Company, LLC, C10 Conveyor area, on or about January 25th, 2024, and times prior thereto: Contract employees were not adequately informed of the hazards and required precautions necessary for safely entering and working in the C10 conveyor tunnel areas. Contract employees entered the C10 conveyor tunnel to conduct chip vacuum operation without precautions and were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
