29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about April 5, 2023, and at times prior thereto, employees were replacing belts on energized exhaust fans without applying locks to isolate the energy, exposing the employees to being caught in the rotating belts and pulleys.
