29 CFR 1910.146(c)(7)(iii):The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person involved: a) Facility (Brew Kettle area) - On or about April 6, 2023, the employer did not document the basis for determining that all hazards in a permit space had been eliminated in accordance with this section. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
