29 CFR 1910.23(c)(2): The employer did not ensure that each stepladder or combination ladder used in a stepladder mode is equipped with a metal spreader or locking device that securely holds the front and back sections in an open position while the ladder is in use. On or about January 12, 2023, in the preparation hangar, employees performing prep work used an A-frame stepladder with spreader devices that did not lock to secure the ladder in the open position.
