29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: Location: 1 Princeton Road, Burlington, MA On or about 12/22/22, the employer allowed an employee to be hoisted on an improvised work platform that was just a wooden pallet to the second story of a new residential construction to perform framing activities. The improvised platform was not a manufacturer approved work platform and was unsecured/unguarded.
