29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard. On or about December 15, 2022, and at times prior thereto, employees were working around an open floor hole on the third level of a new apartment building under construction without the hole being covered and marked or color coded, exposing employees to a fall hazard of over 6-feet to the concrete garage below.
