29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure a periodic inspection was conducted at least annually to ensure that its lockout/tagout program was being followed and any deviations or inadequacies were identified. This condition exposed employees to amputation, crushing and shock hazards associated with an improperly locked out equipment.
