29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: (a) Cut, Bend, Form - Department 1 - Employees used a foot pedal to actuate the Famco 2 pneumatic arbor press, employer machine number SP5, to bend a chair back with legs piece. Employees hand-fed and held the piece, and the point of operation had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about October 27, 2022. (b) Cut, Bend, Form - Department 1 - Four radial arm saws, employer machine numbers S1, S2, S3, and S4, used to cut aluminum, had no guarding on either side of the lower blades, exposing employees to the hazard of getting struck-by a rotating saw blade, on or about October 27, 2022. (c) Cut, Bend, Form - Department 1 - Employees used a foot pedal to actuate the Chicago mechanical power press brake, serial number L7023, employer machine number PB4, to bend some parts, including chair backs. Employees hand-fed and removed each part, and the point of operation had no guarding when actuated by the foot pedal, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about October 28, 2022. (d) Cut, Bend, Form - Department 1 - Employees used a foot pedal to actuate the Chicago Dreis & Krump mechanical power press brake, model 265, serial number L-12354, employer machine number PB2, to crush the upper thread of a bolt. Employees hand-fed and removed each part, and the point of operation had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about October 28, 2022. (e) Cut, Bend, Form - Department 1 - Employees used a foot pedal to actuate the Niagara mechanical power press brake, employer machine number PB3, to bend a metal part. Employees hand-fed and removed each part, and the point of operation had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about October 28, 2022. (f) Cut, Bend, Form - Department 1 - Employees hand-fed aluminum sheets into the Lodge & Shipley Company shear, model 0308-AB, serial number 46918, and actuated this machine by foot pedal. The fixed barrier guard on the machine had a height of 7/8 inch above the table and did not cover the left and right sides of the machine, which would allow employees' fingers and hands to reach the shear blade located 3.5 inches back from the bottom of the fixed guard, on or about October 28, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
