29 CFR 1910.146(c)(9)(iii): Inform the host employer of the permit space program that the contractor will follow and of any hazards confronted or created in permit spaces, either through a debriefing or during the entry operation: (a) On or about April 28, 2022, at or near Milton R. Young Station Minnkota Power Cooperative, 3401 24th Street SW, Center, ND, the contractor did not inform the host employer of the permit space program they would be following and/or implement with identified hazards for employees entering the Plenum duct system to conduct cleaning operations including the removal of fly ash.
