Text For Citation: 01 Item/Group: 001C Hazard:
29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Century Building, B007 Mechanical Room - Three employees did servicing and maintenance work on the domestic water heater system, to include replacing water piping for both domestic water heaters 1 and 2. The system had energy sources, which included hot water and low pressure steam. Each employee did not affix a personal lockout or tagout device to a group lockout device for each energy source before working on the equipment, exposing employees to hazards, such as getting struck-by hot water, on or about February 21, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
