29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) On or about January 13, 2022 - Facility, Landen band strap machine, employees were exposed to electrical, struck-by, and caught-in hazards when the Strapper #1 Lockout/tagout procedure had not been inspected since October 11, 2020. Deficiencies such as, but not limited to, incorrect machine voltage had not been corrected and heat was not listed as an energy source.
