29 CFR 1910.134(f)(2) as referenced in 29 CFR 1926.103: The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: a) Worksite at 621 Main St, Boonton, NJ and corporate office at 649 Catherine St, Perth Amboy, NJ The employer did not ensure that workers who were required to wear a filtering facepiece respirator while removing siding and hand sanding paint containing 4.5%, 5.2%, 6.2% and 8.0% lead and 0.007%, 0.0091% and 0.0313% cadmium were provided with a fit test. Violation occurred on or about 09/21/2021.
