29 CFR 1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(i)-(vii): (a)Hawthorne Ridge Inc., 30 Community Way, East Greenbush, N.Y.: On or about 8/16/2021, and thereafter, the employer did not provide employees with effective training including, but not limited to training on proper fit, limitations, maintenance and care, and medical signs and symptoms preventing the use of the respirator. The employer required Registered Nurses, Resident Assistants, and Licensed Practical Nurses to wear N95 respirators while providing direct care to suspected and confirmed positive COVID-19 residents.
