29 CFR 1910.213(c)(3): Hand-fed ripsaw(s) did not have non kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: (a) Elward Systems Corporation at 680 Harlan St. Lakewood, CO 80214: On or before 7/28/21 the employer did not protect employees from injury in that the guard assembly including non-kickback fingers or dogs were not installed on the Rockwell Unisaw tablesaw [model: 5-66, serial number: KR2018] while in use by employees. This condition exposed employees to amputation hazards. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
