Text For Citation: 01 Item/Group: 001C Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): 1) Prepare for shutdown, 2) Machine or equipment shutdown, 3) Machine or equipment isolation, 4) Lockout/tagout device application, 5) Residual/stored energy release, and 6) Verification of isolation. On or about May 21, 2021, the employer failed to ensure that the application of lockout/tagout procedures were sequentially followed when performing tasks that required the utilization of lock out tag out procedures. Applicable to machinery including but not limited to the low pressure tilt molds 1-7 with energy sources such as electric, pneumatic, mechanical and gravitational. NOTE: The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1517390, citation 1, item 1b, which was affirmed as final order on April 16, 2021 with respect to the same workplace location.
