29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or about 04/17/2021 - Strapper #5 at Line #1 in the #2 position, employees were exposed to caught-in hazards when performing periodic maintenance activities such as, but not limited to, oiling and/or wiping the chain in the Timing Belt Table Drive unit with a hand-held rag and the machine had not been shut down as required in the written lockout/tagout procedure number 80 for the Auto Strappers.
