29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a. First Choice Packaging, Inc. - Fremont, Ohio: On or about February 5, 2021, energy control was not conducted in an order that would ensure the SencorpWhite Thermoforming machine (s/n 18-10499) was isolated from all energy sources before employees performed a mold changeover. Employees were exposed to crushing injuries in that the gravitational energy of the mold was not restrained and rendered safe before employees set up the press. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
