At the US Lumber Group, LLC. outside yard, at 4627 JP Hall Blvd, on or about March 9, 2021, and at times prior, the employer did not review operating instructions, warnings, or precautions listed in the Combilift 8000 Operator's Manual concerning ground operating surfaces, loading instructions, lift heights, and traveling with a load, exposing forklift operators to rollover hazards.
