29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed. (a) On or about February 13, 2021, employee(s) performing electrical work on or around the Russelectric 15,000 volt ATS #2 circuit breaker did not test the electrical circuit to insure that it was in a de-energized state prior to conducting work on the 13,200 volt AC circuit. This exposed employee(s) working at or around the circuit breaker to Arc-flash, electrical shock, and burns.
