29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about October 26, 2020, and times prior thereto, the employer developed a written energy control procedure for cleaning, maintenance, conducting dryer chain adjustments, unjamming floor tiles, and other servicing activities. The employer did not affix a lockout or tagout device when performing cleaning, unjamming and other maintenance activities to isolate the hazardous mechanical energy (i.e. gravity). This practice exposed employees to moving machine parts of the following equipment: (a) In Plant 1, in the Sacmi Press dryer 3 exit elevator and dryer table, counterweight chain; (b) In Plant 1, in the Sacmi Press dryer 4 exit elevator and dryer table, counterweight chain; (c) In Plant 1, in the Sacmi Press dryer 5 exit elevator and dryer table counterweight chain; (d) In Plant 1, in the Sacmi Press dryer 6 exit elevator and dryer table counterweight chain.
