29 CFR 1910.30(a)(1): The employer did not provide training to each employee who uses personal fall protection systems prior to being exposed to a fall hazard. Tri-State Garden Supply, Inc dba Gardenscape, Inc., On or about September 2, 2020, at the address of U856 State Route 66, Archbold, Ohio 43502: the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing hopper cleaning operations from elevated surfaces greater than six feet from the level below.
