29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: a) 2509 South Fourth Operating, LLC, d/b/a St. Monica Center for Rehabilitation and Healthcare located at 2509 South Fourth Street, Philadelphia, PA 19148: On or about June 1, 2020, and thereafter, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees, including registered nurses, certified nursing assistants, and licensed practical nurses, to wear N95 respirators, including 3M brand N95 respirators model # 1860, while providing direct patient care to suspected and confirmed COVID-19 positive residents.
