29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator: a. 1st, 2nd, and 3rd Avenues at 1276 Hanover Street, Manchester, New Hampshire - On or about June 11, 2020, the employer did not ensure that initial fit tests were provided to employees who were required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators when entering and cleaning patient rooms and providing care to patients who were suspected or confirmed positive for COVID-19.
