29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: a) Clark Nursing & Rehabilitation Center, 1213 Westfield Ave, Clark, NJ - Patient Care Units and Special COVID-19 unit: On or about April 16, 2020, and continuing thereafter, the employer required employees, including, but not limited to, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Social Service Workers, and Activity Assistants, to wear N95 tight-fitting facepiece respirators while caring for patients and residents, including those suspected and/or confirmed as COVID-19 positive. The employer did not perform respirator fit testing for all employees required to wear a tight-fitting facepiece respirator.
