29 CFR 1926.1408(b)(4)(ii)(B): The employer did not have a dedicated spotter positioned to effectively gauge the clearance distance: a) Madison Dam Civil Project: On or about July 13, 2020 and at times prior, employees were exposed to electrical shock and burns while placing concrete blocks below the energized 100,000-volt overhead line with a Link Belt TCC 750 crane. The employer failed to provide a designated and dedicated crane spotter to prevent encroachment into the minimum approach distance and accidental contact with the 100,000-volt overhead transmission lines.
