29 CFR 1910.134(f)(2): The employer did not ensure employee(s) using tight-fitting facepiece respirators were fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used: a) At the facility, 2305 Rancocas Road in Burlington, NJ - The employer provided and required the use of respirators at the facility. Employees such as, but not limited to, LPNs, RNs, CNAs and housekeeping staff were not fit tested on approved respirators labeled, on or about 4/6/2020 and after. b) a) At the facility, 2305 Rancocas Road in Burlington, NJ - The employer provided and required the use of respirators at the facility. The employer did not ensure that a nursing supervisor who was provided with and required to wear an N95 respirator, passed a fit test prior to use of the respirator, on or about 4/6/2020 and after. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
