29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): (a) On and before March 11, 2020, the employer did not ensure that group lockout or tagout devices were used during service and maintenance activities in which more than one employee was performing work on a roof-top ventilation fan which was deenergized. This exposed maintenance employees to electrical shock hazards and amputation hazards.
