29 CFR 1910.1200(h)(3)(i): The employer did not ensure that employee training included methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.): (a) The Williams Companies, at 23759 Weld County Road 34.5, La Salle, CO: On and preceding 3/2/2020, The Williams Companies did not ensure that employee training included methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.). Employees performed pigging operations at a compressor facility. During the pigging process, flammable liquids, vapors and gases were released from the receiver and launcher. Employees continued to perform pigging as flammable liquids were drained into open topped cattle troughs and vapors and gases were released from the launcher and receiver. Potential ignition sources were in proximity to the operation. The flammable liquids, vapors, or gases were ignited and a fire resulted. Four gas meters were not utilized by the employees. The employer did not provide effective training to the employees regarding the detection of flammable vapors or gases. This condition exposed the employees to a fire hazard.
