29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Lettuce Processing Area, Lettuce Line Inspection Conveyor: On or about February 1, 2020, an employee in the process of completing cleaning and sanitation operations to the company's food processing equipment located in the lettuce processing area, was exposed to struck-by and caught-in between hazards in that the energy isolating device for the Lettuce Line Inspection Conveyor was not operated in such a manner as to isolate the machine or equipment from the energy source
