29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Building 23, depalletizing robot cell; On or about January 9, 2020, lockout devices were not affixed to electrical and pneumatic isolation points while maintenance operations were performed on the pusher table section of an automated depalletizing robot (Fanuc, model M410-iB, robot # 2). An employee injury occurred, which resulted in hospitalization.
