29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the magnitude of the energy before the authorized or affected employee turned off a machine or equipment: a. On or about 12/12/2019, at the fabrication area, employees were exposed to the inadvertent start-up or release of energy during maintenance of equipment such as but not limited to the Shear Lines and conveyor tables, cutting, bending and coil machines, in that, the energy control procedures did not include the magnitude of all energy sources.
