29 CFR 1910.145(f)(4)(v): Employees were not informed as to the meaning of the various tags used throughout the workplace and what special precautions were necessary: On or about and times prior to October 30, 2019 the employer did not ensure that employee(s) were informed of the meaning of various tags that include the purpose and potential hazards against which proper precautions need to be taken when working with equipment such as, but not limited to, rotary valve "cyclone" machines. This condition exposed employees to caught-in-between hazards.
