29 CFR 1926.502(i)(4): All covers shall be color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard. a. On or about September 9, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through three floor holes on the left and right ends and center of the platform by ensuring that floor hole covers were color coded or marked with the word HOLE or COVER, thereby exposing employees to a fall hazard of approximately 40 feet. b. On or about September 11, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through three floor holes on the left and right ends and center of the platform by ensuring that floor hole covers were color coded or marked with the word HOLE or COVER, thereby exposing employees to a fall hazard of approximately 40 feet. On September 11, 2019, employees were working from a temporary platform located in an elevator shaft. The employee stepped onto an unsupported, unsecured and unmarked floor hole cover on the left end of the platform which caused the employee to fall approximately 40 feet resulting in death.
