29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: On or about 7/30/2019, throughout the production area, employees performing tasks such as but not limited to operating, cleaning, clearing jams, and maintenance work on equipment including but not limited to the cappers and fillers were exposed to struck by, caught in, & amputation hazards where each worker did not apply their own lock and relied on one lock for multiple persons.
