29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Carbo Forge, Inc., located at 150 State Route 523, Fremont, OH 43420: On or about June 17, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not control all hazardous energy sources such as, but not limited to, gravitational before changing blades on the 530 Ton Hydraulic Shear (Advanced Machine Design Company, Model # 530-2230; Serial # 9102) located in the Shear Building. Employees were exposed to struck-by and pinch point hazards that could result in amputations.
