29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about May 28, 2019, at a worksite located at 180 Penniman Road, Orwell, OH, during steel erection activities including, but not limited to, hoisting and landing metal decking bundles on a structural steel building, employees were exposed to fall and collapse hazards of approximately 26.4 feet when the employer did not ensure proper fall protection was implemented and used.
