29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 29, 2019, the employer did not ensure lockout-tagout devices were in place prior to allowing an employee to engage in adjusting a taper press die at the point of operation. This exposed the employee to caught-in hazards.
