29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) South, East, and West side(s) of foundation excavation, while building forms and rebar assemblies: On or prior to 5/9/19, the excavation was not inspected by a competent person. Employees were exposed to cave-in and struck-by hazards while working in an inadequately protected excavation which was approximately 6 to 13 feet deep.
