29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) 221 Sunset Blvd, Chambersburg PA - On or about March 20, 2019, an employee's fingers were crushed by an Amada HFBO hydraulic press brake. The press brake was not shut down, energy isolation devices were not applied to control energy sources, and verification of isolation of all energy sources was not performed prior to changing the tooling. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
