29 CFR 1910.30(b)(1): The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment: On or about 2/20/19, the employer failed to train employees in the proper inspection, storage, and use of ladders, including but not limited to the Louisville 24' extension ladder. Ladders were not inspected for defects prior to every shift they are utilized. Ladders, including but not limited to the 24' Louisville extension ladder, was covered in foreign material (grease, oil, and machine coolant) which had been dripping from the mechanical power presses above, and transferred from employees during use. The ladder also had a bent shoe pad that was covered in foreign material and the slide rail had a crack that was approximately 2.5" in length.
