29 CFR 1910.333(b)(2)(iii): A lock and a tag was not placed on each disconnecting means used to de-energize circuits and equipment on which work was to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section: ESTABLISHMENT: The employer did not ensure through maintaining specific instructions and/or visual observation that a lock and a tag was placed on disconnecting means of the BB1 machine prior to allowing employees to manually assemble a prototype machine to install a power distribution box into the machine. On or about 12/14/2018, an employee inserting a power distribution box unevenly came in contact with the backwall resulting in a short circuit and an employee sustaining an electrical shock.
