On or before December 4, 2018, in the wood shop, the anti-kickback pawls (fingers or dogs) on the Powermatic 10-inch Cabinet Saw (model: 2000; S/N: 131120007853) were removed while an employee was ripping boards at a 45 degree angle. On or about December 4, 2018, an employee received an amputation while using the Powermatic 10-inch Cabinet Saw to rip a board at a 45 degree angle without the anti-kickback pawls.
