29 CFR 1910.333(b)(2)(iv): The employer did not properly verify if the components were positively de energized or in an otherwise safe condition to protect the employees from an electrical shock hazard. On or about October 25, 2018, at the Oryx Lease/Pyote Truck Station off Hwy 2355, employees were exposed to electrical arc flash hazards while reconnecting electrical cables in a VFD Electrical Building without properly verifying that all components had been deenergized.
