29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft: a) B37 Metallizer - On or about September 25 through September 27, 2018, employees were exposed to caught-in hazards due to group lockout/tagout (LOTO) procedures not being utilized when performing servicing and/or maintenance work on the B37 Metallizer. The B37 Metallizer has 4 energy sources consisting of electric, pneumatic, hydraulic, and chilled water. b) B37 Metallizer - On or prior to September 24, 2018, employees were exposed to caught-in hazards due to group lockout/tagout (LOTO) procedures not being utilized when performing servicing and/or maintenance work on the B37 Metallizer. The B37 Metallizer has 4 energy sources consisting of electric, pneumatic, hydraulic, and chilled water.
