29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: a) HVAC Duct Floor Openings on 2nd, 3rd and 4th Floors - On or about August 15, 2018 and at times prior to, the guardrail system installed on HVAC duct floor openings were not equipped with a midrail, thus exposing employees to fall hazards of up to approximately 30 feet.
