29 CFR 1926.20(b)(4): The employer failed to permit only those employees qualified by training or experience to operate equipment and machinery: On or about July 30, 2018, at 1001 S. Miami Avenue, Miami Florida, employees were operating a pneumatic high-pressure concrete pump for the purpose of constructing a high-rise condominium building, wherein the operators of the pump were not trained and qualified to operate the equipment, therefore exposing the employees to a struck-by hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
