29 CFR 1910.23(d)(2): The employer did not ensure the minimum perpendicular distance from the centerline of the rungs,grab bars, or both, to the nearest permanent object in back of the ladder was at least 7 inches: On or about May 1, 2018, employees were exposed to fall hazards when climbing ladder rungs on the side of 20 yard roll-off trash containers and the ladder rungs did not have at least 7 inches of clearance between the ladder rungs and the container.
