29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in, and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 5/13/18, the employer failed ensure the following steps were taken when employees were conducting unjamming activities on machinery, including, but not limited to Line A Cooler "Load On" and "Load Off". (Note: all steps are to be done by an authorized employee(s)): 1) Prepare for shutdown, 2) Shutdown machine or equipment, 3) Machine/equipment isolation, 4) Apply lockout device(s), 5) Relieve potentially hazardous energy sources, and 6) Verify energy isolation.
